How to Maintain Weight Loss After Wegovy: A 5-Step Approach

If you're tapering off or have already stopped Wegovy, the short version is: weight tends to come back when nothing replaces what the drug was suppressing — appetite — so the job now is to build structure around eating that doesn't depend on willpower. That means protein-forward meals, a plan instead of in-the-moment decisions, steady (not crashing) blood sugar, and tracking more than just the scale. None of this reverses the biology of stopping a GLP-1 medication on its own, and it works best alongside your prescriber's plan, not instead of it.

Why this is the actual fight

A systematic review and meta-analysis published in The BMJ in January 2026 (West et al., pooling 37 studies and 9,341 participants) modeled what happens after people stop weight-management medication and found regain is typically fast — for semaglutide and tirzepatide specifically, the drug classes behind Wegovy and Zepbound, average regain ran about 0.8kg a month, trending back toward baseline weight around 1.5 years out. A separate Cambridge-affiliated analysis of the same general question found a more forgiving floor: even after the fastest phase of regain, patients in that data kept off close to a quarter of what they'd originally lost. Read together, the honest range is somewhere between "most of it comes back without changes" and "some of it sticks even if you do nothing" — which is exactly why the changes matter.

The mechanism behind both numbers is appetite, not the diet label someone follows. Wegovy works partly by slowing stomach emptying and quieting hunger signals in the brain; stop the drug and that signal usually returns close to where it was before, which is what regain research is actually measuring. So the practical question isn't "which diet is best" in the abstract — it's "what replaces appetite suppression well enough that eating less doesn't require constant willpower."

Where a structured, lower-carb approach fits

This is the gap a well-formulated ketogenic-style pattern is reasonably good at filling, even though it wasn't designed with GLP-1 users in mind. Protein and fat are both more satiating per calorie than carbohydrate for most people, and steadier blood sugar (the kind a lower-carb pattern tends to produce) means fewer of the hunger spikes that follow a carb-heavy meal a few hours later. In the Virta Health / Indiana University Health continuous-care study of a well-formulated ketogenic diet, participants lost an average of roughly 12% of body weight by 12 months, with retention in the program holding at 83% at one year — a different population (people with type 2 diabetes, not GLP-1 users) and a different mechanism than semaglutide, but the same underlying target: appetite and blood sugar, managed through food rather than a drug.

None of this is a claim that a diet "replaces" Wegovy, and no study directly compares the two as a maintenance strategy head-to-head — that research doesn't appear to exist yet. What the data supports is narrower: appetite regulation is the thing that matters most once the drug's effect fades, and a structured, protein-forward, lower-carb pattern is one of the better-studied food-based ways to manage it.

The 5 steps, in practice

1. Protein first, every meal. Not as an afterthought — as the first decision. A palm-sized portion or more at breakfast changes how hungry you are by mid-morning more than almost anything else on this list.

2. Plan before you're hungry. The drug used to make "what am I eating" a non-issue. Now it isn't, so the plan has to do that job — decide meals ahead of time rather than deciding in the moment, when hunger (not you) is doing the negotiating.

3. Keep carbs low and steady. Big carb swings — a very restrictive day followed by a loose one — tend to produce the appetite spikes this whole approach is trying to avoid. Steady beats strict.

4. Watch more than the scale. Regain research shows weight comes back in a pattern, not a straight line — a stair-step of small stalls and drops. Energy, hunger levels between meals, and how clothes fit are signals that move even during a scale stall.

5. Keep your prescriber in the loop. How fast a dose is tapered — not just whether it's stopped — affects how quickly appetite returns, and that's a call for the person managing your prescription, not a diet change made independently.

Skip the decision next time

Keto Coach builds the daily protein-first, planned-meal structure above into an actual day-by-day plan, so "what do I eat today" isn't a decision made from hunger.

Steps

  1. Put protein first at every meal, not last — it's the single biggest lever for appetite once the drug isn't doing that job for you.
  2. Replace 'eat less' with a plan — decide meals ahead of time so you're not negotiating with hunger in the moment.
  3. Keep carbs low and steady rather than cutting hard and rebounding — steadier blood sugar means fewer spikes in appetite.
  4. Track non-scale signals (energy, hunger levels, clothes fit) alongside weight — regain research shows the scale moves in a stair-step, not a straight line.
  5. Loop in your prescriber before any dose change or stop — tapering strategy affects how fast appetite comes back, and that's a medical decision, not a diet one.
